Unfortunately, Anthem sent this letter in error and is working to correct as soon as possible. For now, we want to confirm that your TriHealth provider is still In-Network with your Anthem insurance. Please do not cancel any appointments or start searching for a new physician without consulting your insurance first.

How do I request a price estimate for my test/procedure?

To request a price estimate for services at one of our hospital or ambulatory facilities, please contact a financial counselor.

What information do I need to provide when requesting pricing information?

You will need to provide the following:

  • Facility where services will be completed
  • Name of procedure
  • CPT code (for surgical procedures)
  • Insurance name/policy number (if available)

Will my pricing estimate include all expected out-of-pocket cost associated with my procedure?

No, pricing estimates will include the hospital facility charges only. The estimate will not include any professional fees, such as for physicians, radiologies, anesthesiologists, pathologists, etc.

How do I obtain pricing information for my physician charges?

Please contact the physician’s office directly for professional charge estimates.

In addition to the hospital facility statement, what other bills can I expect to receive for my services?

You may receive professional bills including but not limited to the following:

  • Anesthesia
  • ED physician - QESI
  • Pathology – Southern Ohio Pathology
  • Radiology
