How to Participate
Participation in this program is completely voluntary and at no cost to you. Results are private and your employer will not have access to your individual result.
1. Check Eligibility
If you have a diagnosis of any of the following there is no cost to proceed with genetic testing and receive a medical action plan:
- Depression
- Anxiety
- Bipolar Disorder
- OCD
- Chronic Pain
- High cholesterol
What if you do not have a covered diagnosis, but are still interested in PGx testing? Your employer may expand this program over time. To learn more about PGx testing outside of this program please visit, Pharmacogenomic(PGx) | TriHealth.
2. Request then Complete Enrollment Forms
- Please fill out this form to request the following two forms:
- Pharmacogenomics Information Form
- Consent Form
- Review, complete, and return these forms to PGxTest@TriHealth.com. These forms are required for enrollment.
3. Collect Sample
- Once your forms are reviewed and you are approved, an electronic medical record/MyChart order will be placed for your Pharmacogenomics test.
- OneOme, TriHealth's business partner for the DNA testing, will ship a saliva collection kit to the address you provided in the pre-test information form.
- Using the instructions provided in the kit, you will collect your cheek swab sample and return the kit by mail.
4. Receive Result
- Once the lab receives your sample, it will take about 10 days for the results to come back to you through your TriHealth MyChart.
- A TriHealth pharmacist will review your result, and your current medications. The Pharmacist writes a medication action plan that will also be visible in your MyChart and sent to your primary care provider.
Note: It is important not to change or stop taking any medications until you speak with your provider.
5. Take Action
- If any of your current medications are included in your medication action plan written by the pharmacist, contact the provider that prescribed this medication for further discussion.
- You also have an option to speak with a TriHealth Pharmacist about the medication action plan. A link to schedule will be provided when your result is available.
- It is important to understand that the pharmacist can ONLY review the report and your medications, but cannot make changes to your prescribed medications. Changes to medications can only be made by the treating provider.
