Primary Care w/Geriatrics – Medical Director – PACE of Cincinnati in Cincinnati, Ohio
PACE of Cincinnati is a large population, highly collaborative, interdisciplinary practice that insures patients get the right level of care in the right setting at the right time.
PACE, Program of All-Inclusive Care for the Elderly, is a managed care model that provides participants with all their needed health care, medical care, and ancillary services in acute, sub-acute, institutional, and community settings. An inter-disciplinary team directs and provides the care of their participants. Services include primary and specialty care, adult day health services, personal care services, inpatient hospital care, prescription drugs, occupational and physical therapies, and nursing home care.
The Medical Director position is a blend of clinical practice and administrative functions and works in partnership with the PACE Senior Director. Candidates will possess strong leadership skills and a passion for working with elderly patients. The ideal candidate will embrace a holistic model that addresses medical as well as social determinant needs in order to keep frail and complex medically impacted seniors living safely at home.
Duties:
- provides leadership and oversight for all PACE primary care and clinical activities.
- is an integral member of the PACE leadership team and expected to participate (under the direction of the Executive Director and Board of Directors) in activities aimed at driving the strategic path of the PACE program.
- in collaboration with the Executive Director, Center Director as well as all Clinical and Quality leadership, plays a key role in creating an effective PACE culture, operations, and interdisciplinary team approach to care.
- oversees participant primary care protocols and participant clinical outcomes and directs the Quality Improvement (QI) program.
- is responsible for managing relationships with outside contracted medical providers of PACE services, such as hospitals, skilled nursing facilities, physician specialist providers, and ancillary services providers.
- may be responsible for the supervision of PACE primary care providers including physicians and advanced providers.
- attends and participates in PACE of Cincinnati leadership meetings and Board of Director meetings as needed
- will be able to apply evidenced based guidelines and utilization management principles to participant-centered care plans.
Requirements:
- MD or DO with current license to practice medicine in the State of Ohio
- Board-certified in internal medicine or family medicine with advanced certification in geriatrics; prefer experience, training, and/ or certification in palliative care and hospice.
- Three or more years of experience preferred in a healthcare leadership position.
- Current DEA registration
- One year of direct care experience with a frail, elderly, or long-term care population’s physical, mental, emotional, and social needs.
- Demonstrates ability to work in an interdisciplinary team setting effectively; has experience in team development, team dynamics, conflict management, and fostering strong interdisciplinary culture.
- Demonstrates skills and knowledge in primary care provider competency requirements, including effective skills in physical assessment and chronic disease management for frail older adults.
- Staff privileges at PACE of Cincinnati contracted hospitals and nursing homes.
- Medically cleared for communicable diseases; have up-to-date immunizations and vaccines prior to engaging in participant care.
- BLS certified
Position/Panel Overview
- full-time, five-day workweek/40-50 hours weekly
- rotating, after-hours call; available evenings, nights, weekends for telephone call
- several daily office visits; occasional home visits or hospital visits
- panel of 60-100 patients consisting of low-income Medicare and Medicaid duly-eligible frail seniors that meet nursing home eligibility
