What happens if I can’t make my appointment?

If you can’t make your appointment, call the office at least 24 hours in advance to cancel.

New Patient Attendance Policy

If you do not call to cancel your appointment, you will risk dismissal from the practice.

Existing Patient Attendance Policy

Failure to cancel your appointment will count as a “No Show”. Four “No Show” appointments in a rolling calendar year will result in dismissal from the Faculty Medical Center.

Are my medical records confidential?

All information (financial, medical, and family) is part of the patent’s Electronic Medical Record (EMS) and is kept completely confidential. This information will not be shared with employers, family members, or others without the written authorization of the patient.

The EMS records are connected to all TriHealth Hospitals and Physicians, so if you need a referral to a specialist or another location, your medical records will be accessible.

What happens if I need a referral to a specialist?

During your visit, your Primary Care Provider may find it necessary to refer you to another office or medical provider for additional screenings, evaluation or care.

It is our goal to assist to the greatest degree possible with the scheduling of any referrals. In order to be able to guide our patients through the entire process we ask that our patients schedule all of their referrals with the help of our office personnel before they leave the office.
