TriHealth announced today they have entered into a new clinical collaboration with CVS/Pharmacy and MinuteClinic. The affiliation will increase access to care and help improve medication adherence for patients.
“TriHealth and CVS Health have established a clinical affiliation given our mutual goal of improving the lives and health outcomes of the patients we serve,” said Steve Mombach, VP of Ambulatory Services for TriHealth. “Through this collaboration, we believe our patients will have enhanced access to pharmacy care and coordinated, primary care support.”
Many patients often struggle with prescription management – especially those with chronic diseases on multiple medications. The collaboration with CVS Health will support providers’ care plans, communicating critical patient and pharmacy care information in order to help TriHealth improve health outcomes for patients and lower medical costs.
Through these clinical affiliations, CVS Health will provide prescription and visit information to the participating health care organizations through the integration of secured electronic medical record (EMR) systems, which will help enhance clinical care for patients served by the partnering organizations. Through this clinical collaboration, TriHealth patients will continue to have access to clinical support, medication, counseling, chronic disease monitoring and wellness programs at CVS Pharmacy stores and MinuteClinic. In addition, CVS Health will provide prescription and visit information by enabling communications between our secured electronic health record systems.
“Our top priority is our patients. As a Patient-Centered Medical Home, primary care physicians view themselves as the quarterback for our patients’ care,” said Diane Dolensky, MD, Medical Director of TriHealth Physician Practices. “When the immediate needs of our patients lead them to seek care outside of our practice, we want to provide options where we have quality, collaboration and continuity of care while still knowing we’re all on the same team. By partnering with CVS, we can provide our patients with additional supplemental care options without sacrificing our ability to be a consistent champion for our patients’ overall health care.”
“In this era of health care reform, we are pleased to work with TriHealth to develop collaborative programs that enhance access to patient care, improve health outcomes and lower health care costs in the communities they serve,” said Troyen A. Brennan, MD, Chief Medical Officer, CVS Health. “By integrating our electronic medical records and information systems, we will collectively have a more comprehensive view of our patients, which can aid in health care decision making and help ensure patients adhere to important medications for chronic diseases.”
CVS/pharmacy currently has more than 7,800 retail pharmacy locations across the U.S. where CVS pharmacists provide counseling to patients to help them be adherent to their chronic disease medications. In addition, MinuteClinic also plays an important role by providing patients with timely, affordable and high-quality walk-in health care. There are more than 900 MinuteClinic walk-in medical clinics available at CVS/pharmacy retail stores. MinuteClinic locations are open seven days a week, offering evening hours with no appointment necessary.
