**Recommendations are for non-immunocompromised patients. Patients that are immunocompromised should speak to their physician regarding recommended COVID-19 Vaccine schedules.

Completed 1 Updated COVID-19 Vaccine 2023-2024 Formula (Omicron variant XBB.1.5)
Adults 65 years and Older

The CDC’s new recommendations allow for an additional COVID-19 Vaccine 2023-2024 Formula for those adults 65 years and older. Those adults 65 years and older that have received one COVID-19 Vaccine 2023-2024 Formula can receive an additional vaccine at least 4 months from their COVID-19 Vaccine 2023-2024 Formula (booster).

What to Bring With You

  • Insurance Card: There is no out-of-pocket cost for receiving the vaccine, but you will be asked for your insurance information
  • Driver's License
  • Vaccination Card
Schedule a Pediatric Patient ages 12 years to 17 years**

Pediatric patients ages 12 to 17 years are also recommended to receive COVID-19 Vaccine 2023-2024 Formula. If your child has not received one COVID-19 Vaccine 2023-2024 Formula or is unvaccinated against COVID-19, please call 513 86 COVID (513 862 6843). **Patients under 12 are encouraged to contact your pediatrician to schedule an appointment.
